Grades; Grade Point Averages; Class Rank; Academic Dismissal; Hours Attempted, Earned, or Transferred; Student Account Balances; Financial Aid Received or Pending; Employment Status or Employer; Disciplinary Action or Law Enforcement Records; Personal Counseling Records or Academic Accommodation Status; Medical Records; Student Number or Social Security Number.
I understand that any violation or release of this information is a severe offense and will lead to immediate dismissal from my work-study or college roll position at the University.
I understand that sharing or disclosing information related to any exams, quizzes, tests, or other assessments administered in the Academic Support Center is a violation of the University's Academic Honesty policy and will lead to immediate dismissal from my work-study or college roll position as well as potential disciplinary action by the academic department responsible for the assessment in question.
I further understand that sharing, disclosing, falsifying, or otherwise altering information, either within a student's file or on a university database, violates university policy and federal regulations and will be subject to disciplinary action. Students who are found guilty of severe violations of university policy are subject to dismissal.
